Boarding and Facility Agreement.
Thank you for trusting your pet with Family Animal Medicine. Our boarding facility is indoor, air-conditioned and heated. All boarding dogs are walked a minimum of four times each daily. All boarding animals are fed Purina Prescription EN dry pet food (unless provided by client). If you provide your own food for your pet, but it runs out before the end of their stay, they will be fed our FAM kennel food for the duration of their time boarding. Pets with complex feeding instructions, prescription or medical diets, are subject to additional charges for medical boarding as deemed by the veterinarian. Pets here for daycare and grooming are not fed except upon request. We now have 3 types of boarding, Regular Boarding for pets with no medical needs, Medical Boarding Level 1 for pets who require only simple medication administration or minimal medical observation/care, and Medical Boarding Level 2 for pets who require advanced medical observation, insulin administration, controlled or special medications. Our doctors determine the level of medical boarding required. The charges for the daily medical boarding will now cover the administration of the insulin/medications while here so there are no separate charges for individual doses/injections. All boarding, day boarding, and grooming animals must have a current yearly exam with a veterinarian, must be current on vaccines (Distemper, Rabies, Lepto, Bordetella/Kennel Cough/CIV), and have a negative fecal within the last 12 months. Any boarding, day boarding, and grooming pets with fleas or ticks will be treated at the owner’s expense. If a pet refuses our dry food, or their own food from home, our canned food and diet supplements will be provided at the owner’s expense. If your pet appears stressed or anxious while boarding, we will treat with appropriate medications for their comfort at owner’s expense. If your pet experiences GI upset while boarding, we will treat with the appropriate medications at the owner's expense. Pets that experience medical problems while boarding will be examined and treated at the owner’s expense. We will attempt to contact you via your emergency contact numbers if any concerns or problems arise during your pet’s stay. However, if there is a medical emergency, the doctor will treat at their discretion until we can reach you. Playtime and additional exercise can be added at a charge of $5 per 15 minute session. Dogs staying five or more nights will receive a bath at no charge the day of discharge, however pick up will be at least noon. We provide bedding, food and water bowls, etc. We ask that you not bring any items for your pet other than medications or special food. For the safety of all pets, and to prevent the loss of items, we are no longer accepting bedding, toys or other items from home. We take strong precautions to prevent the transmission of contagious disease and parasites in our facility by requiring vaccinations and fecal testing for the pets who stay with us; disinfecting and sanitizing all areas of our facility, and monitoring all animals under our care. However, contagious diseases and parasites can still be spread despite these precautions. Owners boarding with us accept this risk and are responsible for treatment of their pets while boarding or after their stay. The acceptance of this boarding agreement by your signature on the boarding intake form will remain in effect and applicable to all of your pets under our care until revoked in writing.
